The forms will vary based on what type of passport you are requesting. Express Passport will give you a check list of forms you must fill out and send in once you have selected and paid for a service. A common hold up of receiving a passport is incorrectly filling out a government form. This is where Express Passport helps. New Passport Form (Form DS11)
You must use this form if: You have never had a U.S. passport You are less than 16 years old

Lost, Stolen or Damaged Passport (Form DS11 and DS64)
If your passport is Lost, Stolen or Damaged, you must fill out both the DS-11 and DS-64 forms.

Passport Renewal (Form DS-82)
You must use this form if you meet all of the following criteria: Your prior passport was issued when you were 16 or older Your prior passport was issued within 15 years. Your prior passport is within your possession. Your prior passport is not mutilated or damaged

Name Change (Form DS-5504)
You must use this form if: You need to change the name on your passport. This form only applies if your passport is less than a year old.

Passport Pages (Form DS-4085)
You must use this form if: You need to add pages to your passport. This form only applies if your passport has at least one year left of validity.

Child Passport (Form DS-11 and DS-3053)
If your child is 15 years of age or under and you must use the Statement of Consent Regarding a Minor ( DS-3053) form if: One of the legal guardians or parents cannot be present at the acceptance agent
